This murmur was created by a small muscular VSD (also known as swiss cheese defect). It was graded 2/6 on physical exam and sounds more like a "classic" newborn VSD murmur than the previous two. This murmur was not noted on the initial examination, but on the second exam, it was easily appreciated when the infant was quiet.
